Hypertriglyceridemia can be defined as the presence of a significant amount of triglycerides in the blood and severe gestational hypertriglyceridemia is defined as plasma TGs >1,000 mg/dL. The risks of severe hypertriglyceridemia to the mother include acute pancreatitis, hyper viscosity syndrome, and preeclampsia. Gestational Hypertriglyceridemia is a rare entity and needs to be developing awareness regarding the disease. The most important fact is that pregnancy induces changes in lipid fractions and their level rises. There is 2-to-4-fold rise in plasma triglycerides. Usually, these changes barely cause any significant symptoms unless the levels rise above the 95th centile. The rare subgroup of pregnant individuals that develops severe HTG, defined as plasma TG greater than 11.4 mmol/liter (1000 mg/dl), may suffer from an increased risk of acute complications and may have a chance of expressing hyperlipidemia in the future. We, hereby present a case of 24 years old obese, diabetic pregnant female with incidental finding of hypertriglyceridemia. Informed consent was taken from the patient.
